These programs are designed to improve patient health and wellbeing, decrease unnecessary emergent care and acute care hospitalization, and restore patients to a higher level of functioning. Our programs utilize a multidisciplinary team approach to develop and tailor a plan of care to better meet the patient’s individual needs. Through the use of evidence-based clinical pathways and structured education tools, clinicians are able to identify problems, report changes to the physician, provide immediate education and patient support, and communicate between team members.

Cardiac Hospital Admission Management Program

Premier Homecare offers a unique cardiac care program for home health patients. Our Cardiac Hospital Admission Management Program is designed to reduce and/or prevent hospital readmissions in patients with cardiac dysfunction. The program incorporates the use of daily telehealth monitoring, structured cardiac rehab by licensed therapists, and evidence-based patient education and behavior modification to improve patient self-management skills, increase awareness of disease management, and decrease utilization of emergent care and acute care services.
Our multidisciplinary team approach includes skilled nurses, physical therapists, occupational therapists, medical social workers, and home health aides. The staff of Premier Homecare work with the patient’s primary physician and consulting physicians to coordinate health services to provide comprehensive health services.
Conditions appropriate for the program include congestive heart failure, coronary artery disease, HTN, vascular insufficiency, malaise and fatigue associated with heart disease, dyspnea, and/or recurrent edema.

Pulmonary Management Program

Premier Homecare’s Pulmonary Management Program is a unique program that combines intermittent skilled nursing services with structured therapy to provide the patient with home-based pulmonary rehabilitation services. This program is appropriate for homebound individuals with COPD, asthma, pneumonia, or other chronic pulmonary disease that are unable to participate in traditional outpatient pulmonary rehab programs. 

 A multidisciplinary team approach is implemented, and a plan of care is tailored to meet the unique needs of the patient. Standard services available to the patient include skilled nursing, physical therapy, and occupational therapy. Additional services are available as needed and include speech therapy, medical social services, and home health aide services. Evidence-based clinical pathways and structured patient education materials are used to identify health concerns, report changes to the physician, provide immediate education and patient support, and communicate between team members. 
Upon completion of this 9-week program, patients report an increase in ease of breathing, increased ability to recognize symptoms of disease exacerbation, improved compliance with medication regimens, increased activity tolerance, and an overall improvement in quality of life.

Wound and Ostomy Program

Premier Homecare’s wound, ostomy, and continence (WOC) program provides patients with clinical and consultative services to improve overall health outcomes. This service is provided by a board-certified wound, ostomy, and continence nurse (CWOCN), or a nurse who is extensively trained in specialty wound, ostomy, and continence nursing. The CWOCN serves as a liaison between the patient, staff, and physician. The CWOCN will develop a treatment plan that is tailored to meet the patient’s individual needs.  Patients who may be appropriate for this program include: ostomy patients with peristomal skin complications, ostomy patients who require more than two pouch changes weekly, patients with continent diversions requiring teaching, and patients with acute or chronic wounds that require observation/debridement.

Urinary Incontinence Program

Memory Care/Psychiatric Nursing Program

Premier Homecare’s team of professions work collaboratively with patients that have cognitive impairment. Our team uses a blend of compassion, care, and training to work with clients who face the challenges of cognitive and memory deficits. Our goal is to build effective communication methods with patients and caregivers while implementing strategies and interventions to promote independence for as long as possible. When difficult to manage behaviors and/or personality changes occur, our team of mental health nurses can assist in counseling and proving strategies to address behavioral issues. Highly skilled home care clinicians provide patient education materials focused on the needs of each individual. 

Mental health is an important component to the overall health and wellbeing of our patients. At Premier Homecare, mental health services are provided in the comfort of the patient’s own home. Home offers a safe and effective environment to learn how to cope with challenging behavioral and mental health issues. The mental health registered nurse (RN) will treat the mental and physical needs of the patient by using a patient centered approach to provide counseling, medication monitoring, observation and assessment of mental health and stability, and education regarding disease progression.   
In an effort to assist patients in recovery at the highest level, our team of psychiatric nurses work closely with the family members and caregivers of our patients.

Infusion Therapy Program

Premier Homecare provides home-based infusion therapy services for a variety of specialty intravenous solutions for a variety of medical conditions. Our agency specially trained and experienced Registered Nurses provide the service to the patient. Some infusions, such as routine antibiotic therapy, are safely administered by caregivers and/or patients after instruction by the nurse. Other infusions, specialty infusions such as IVIG or Biologics, require the administration over the course of several hours by the RN. For specialty infusions, the RN administers the medications and remains at the bedside to monitor the patient throughout the administration process.

Low Vision Rehabilitation

Premier Homecare’s low vision program was developed by clinicians to assist patients with low vision improve their ability to function independently and safely at home. Our multidisciplinary team members are specially trained to assess safety and needs due to low vision issues. Interventions will not restore lost vision; however, they will enable the patient to use their remaining vision more effectively while performing the activities of daily living.
